Visit www.nurseaidetesting.com. Scroll down until you find the blue
Popular Links box located on the right side of the page. Click on the Online Recertification form within that box. Complete the form accurately and thoroughly, then click Submit.

STEP TWO

Submitting this online form is only the first half of the online recertification submission process. Please call the number 1-877-262-9259. Choose option #2 and leave your Name and Social Security Number (SSN) as requested. This information will be matched to the information you have already submitted online. This extra step is provided as an extra layer of security in the protection of your personal information. If you do not call and complete your submission, we will be unable to process your request.

Please allow 5 business days for your information to be processed. Once it has been processed you will receive an email that may be found in your spam or junk folder. The email will be from INATE - inat@siu.edu.

INATE receives this information as a download file and will check the Health Care Worker Registry to determine if you are active or inactive as a CNA. If you are an active CNA, you will receive an email confirming this. If you are inactive, you
will receive an email containing several attachments:

  • a letter (Word document) that is addressed to you and contains important information about this process.
  • INATE Test Taker Instructions – these are step by step instructions that you will need to follow to create an account in INATE so that you may find, schedule and pay for a seat to take the written test.
  • Important Information for Recertification – is a document that contains information regarding eligibility, time frame and what to bring to the test, fees, test sites, results, rescheduling and how to stay active as a CNA.
  • Types of Testing – contains information on the various types of testing that is provided.
  • FAQs for CNAs – This document is provided by IDPH and provides answer to certain questions that are frequently asked such as: certificate number or copies of certificates, expiration, update of employment history, active status, private duty, transferring, searching name in Health Care Worker Registry, name change, results on registry and other important information.

These steps must be followed in order as you will need the letter/documentation provided to you to understand what your next step will be. Once INATE processes your request, we will submit your name on a roster in our online registration program. You will automatically receive an email from support@dxrgroup.com letting you know you can now register for testing (this is where the INATE Test Taker Instructions will be needed). Once this account is created you will be able to schedule a date/time/place to take the written test.
